Artistè Foundation – Story Submission Consent & Privacy Notice (Legal Form)
By submitting your personal information and/or personal story (“Submission”) through the Artistè Foundation website, you acknowledge and agree to the following terms:
1. Collection of Personal Information
The Artistè Foundation (“we,” “our,” “the Foundation”) may collect the following information as part of your Submission:
1.1 Personal identifiers you voluntarily provide (e.g., name, email address, phone number).
1.2 Demographic information offered at your discretion.
1.3 Written, audio, or visual content that you choose to share.
1.4 Any additional information submitted through online forms, attachments, or communications related to your story.
You are not required to provide identifying details and may submit anonymously.
2. Purpose of Collection
Your information is collected for the following purposes:
2.1 To review, assess, and manage your Submission.
2.2 To contact you regarding your Submission, if necessary.
2.3 To maintain internal records, research, and program development.
2.4 To determine whether you have provided consent for your story to be used in publications, advocacy, programming, or media (see section 4).
Your information will not be used for any purpose not described in this notice without obtaining your explicit consent or unless required by law.
3. Confidentiality and Data Protection
3.1 Your Submission will be stored in secure, password-protected systems accessible only to authorized Foundation personnel.
3.2 Personal identifiers may be separated from your story for additional privacy protection.
3.3 The Foundation implements reasonable administrative, technical, and physical safeguards to protect against unauthorized access, disclosure, alteration, or destruction of your information.
4. Consent for Use of Story
You may choose one of the following levels of consent when submitting your story:

4.1 Full Consent: You authorize the Foundation to use, reproduce, publish, or distribute your story (with or without your name) for programming, awareness campaigns, advocacy, publications, websites, and media.

4.2 Partial Consent: You authorize use of your story only in anonymized form, without any identifying information.

4.3 No Publication Consent: Your story will be viewed internally only and will not be published, shared externally, or used in programming.
Your selection will be documented at the time of submission.
You may withdraw or change your consent at any time by contacting us in writing.
5. Withdrawal of Consent and Deletion
5.1 You have the right to request:

Correction of your personal information

Deletion of your story and/or personal data

Withdrawal of previously granted consent
5.2 Upon receiving a written request, the Foundation will take reasonable steps to delete your information from our active systems, except where retention is required by law.
6. Sharing of Information
6.1 We do not sell or trade personal data to third parties.
6.2 Information may be shared only under the following circumstances:

You have given written consent

Sharing is legally required (e.g., court order)
